Maid of Sker takes place in a hotel on the fictional Sker Island in Wales. The real-life Sker House, near Porthcawl, made famous by The Maid of Sker novel by R.D. Blackmore, as well as the Welsh folklore tale Y Ferch o’r Sger, were some of the inspirations behind the plot and setting.

Reimaginings of famous Welsh hymns feature in the game, including Calon Lân (A Pure Heart), Suo-Gân (Lullaby) and Ar Hyd Y Nos (All Through the Night), all performed by Welsh singer Tia-Rhian Kalmaru.

Other Welsh references appear, such as bunting featuring the flag of Wales and uses of Cymraeg, the Welsh language.
